Metastatic lymph nodes in carcinoma of the oral cavity and their Bleomycin sensitivity.

Effects of Bleomycin (BLM: 60-105mg, I. V.) on metastatic lymph nodes of cervical region from carcinoma of the oral cavity was examined clinically and histologically in 18 patients with total 40 metastatic lymph nodes. Level of involvement by the UICC of these nodes was mostly L-1 and L-2. These cases were classified by clinical effects of BLM on metastasis into: 5 with a good effect (++), 5 with a slight effect (+) and 10 of no effect (-). According to Shimosato's grading system, histological effectiveness in the group of a good clinical effect was good (Grade IV:1, III:5, II b:4, and II a:3), while that in the slight clinical effect group varied (Grade III. 2, II b:4, II a:5, I:1, and 0:1) and that in the no clinical effect group was very slight (Grade I:4, and 0:10). Thus, a relation between clinical and histological effects of BLM on metastasis was found in some degree. Histological effect in nodes with an "intranodal" tumor growth was found to be more than that in nodes with an "extranodal" one (Intra: Grade IV:1, II:7, II b:5 out of 26.
