A case of progressive extramammary Paget's disease showing tumor type.

A case of extramammary Paget's disease with tumor formation was discribed. The patient was a 82-year-old female and had taken therapy under the diagnosis of tinea cruris during about 9 months. At her first visit to us, the lesion developed a hard tumor about 5cm in diameter with erythema on her right vagina. The surface was smooth and partialy covered with necrotic tissue. Inguinal lymphonodes on both sides were palpable. The histological findings of a biopsy specimen from this tumor showed the mass of Paget cells. The diagnosis of extramammary Paget's disease was made. The lesions were resected with at least 3cm-wide margins and Inguinal lymphonode dissection were peformed. We examined this case by histochemical staining, immunohistochemical studies and electron microscopic study. Invasive tumor cells of this caseas well as Paget cells of extramammary Paget's disease reacted equally to antikeratin monoclonal antibodies which stained simple epithelia specifically and detected keratins with low molecular weight. Histochemical findings may provide the evidencethat advanced extramammary Paget's disease are related originally to the sweat gland epithelial cells.
