Findings in the intrahepatic portal vein of patients with hepatocellular carcinoma(HCC) by percutaneous transhepatic portography(PTP) compared to those of the operative specimens.

We examined the findings by PTP of the intrahepatic portal vein of patients with HCC, and compared them with findings of the operative specimens.The findings by PTP included compression, tapering obstruction, interrupted obstruction, filling defect in the portal vein, and focal defect in the sinusoidal phase. Findings by PTP were mostly of interrupted obstruction and filling defects in the portal veins in patients found to have a tumor thrombus in the portal branch in the operative specimen. For findings of interrupted obstruction and filling defects in the portal vein, the sensitivity of detection by PTP of the tumor thrombus was 88%, specificity was 94%, and accuracy was 93%. The findings of compression and tapering obstruction were found to be simple dislocation by the tumor except in one case.Selective PTP and PTP over time, observed in several films, showed blood circulation in the portal vein and changes in the intrahepatic portal veins surrounding the tumor in detail.Findings by PTP were useful both for the detection of a tumor thrombus in the portal vein and for the choice of the treatment in patients with HCC.
