A case involving a 37-year-old man who suffered from a subarachnoid haemorrage due to a ruptured aneurysm arising from a fenestration of the basilar artery was reported. Previous reports were also reviewed. Hemodynamic mechanisms and local histological fragility were considered to be causative factors. Stereoscopic magnification angiography with an oblique projection was a useful technique for the diagnosis of the aneurysms.
