• The state of HBV DNA in the liver of 39 HBV-related liver disease patients and the clinical features were investigated in order to find out when and how HBV DNA integration occurs.<BR>Of 34 HBV carriers (non-cancerous tissue); 3 had only free forms, 20 had both free and integrated forms, 10 had only integrated forms and 1 had neither free nor integrated form, all of the integration were randomly patterned. In only one case having hepatocellular carcinoma, clonal integration was observed.<BR>Integration was detected regardless of the histological activity. Integration was observed in 18 out of 20 HBeAg positive patients, and this indicates that it occurs prior to seroconversion. There was no relation between exacerbation and HBV integration. Most of the adult HBV carriers were observed to have had HBV DNA integration. These findings suggest that integration of HBV DNA occurs spontaneously to the hepatocyte of HBV carriers.
