Studies of End Colostomy for Continent Stoma with Bowel Smooth Muscle Grafting.

For continent stoma, we performed end colostomy experimentally with bowel smooth muscle grafting. We confirmed that clinical application is possible based on the result of pressure test, blood flow, electromyogram and histological findings.<BR>We clinically applied this in 46 cases (free graft 30 cases, pedicled graft 16 cases). We achieved generally satisfactory results with regard to clinical course, internal pressure of the graft, findings on enema studies, and histological findings, although we irrigated in some cases.<BR>For functional stoma, we consider the pedicled graft to be more advantageous than free graft. In pedicled transplantation, " how they should be denerved is important". Now, this method is considered to be clinically useful, but longer follow up is still needed to check the change in smooth muscle.
