Follow-up colonoscopy after resection of colorectal cancer.

For early detection of the newly developed neoplastic legions, 124 patients, who had been resected colorectal cancer, were followed by repeated colonofiberscopy after initial operation. Thirty patients (24.2 percent) developed new adenomatous polyps. Thirty-eight percent of patients who initially had cancer and associated adenomas or cancers (group A) subsequently developed new adenomas compared to 13 % of patients who had single colon cancer without adenomas at the time of primary operation (group B). The size of newly developed adenomas was 5 mm or less in 92 %. Seven metachronous cancers were found in six patients (4.8 %). In six of these seven metachronous cancers, the lesions were limited to the mucosa or the submucosa. The occurrence rate of the metachronous cancer in the group A tended to be higher than that in group B. Based on the results of this study, longterm follow-up by colonofiberscopy is necessary after resection of colorectal cancer. Because the occurrence rate of neoplastic legions in the group A is higher, these patients undergo colonoscopy annually, and the patients of the group B request follow-up colonoscopy once every two years.
