In the reconstruction of injured tissues, the most important point in yielding a high survival rate is to avoid blood flow disturbance by accurate angiostomy. In this point, microsurgery makes a tremendous contribution to reconstructive surgery. However, it has been difficult to anastomose peripheral vessels 0.5 mm or less in diameter. This has caused a severe decline in the survival rate in the reconstruction of fingertips. Recently, the technique to anastomose super-micro vessels has been established through the development of high-resolution operating microscopes and super-micro sutures based on the advanced industrial technology of Japan. This has driven forward vascular surgery to a new frontier. In this study, we introduce a case that became recoverable with “super-microsurgery” and discuss the effectiveness, evolution, and application potential of the technique.
