家族のケア力を高める看護援助に関する研究

本研究の目的は,家族が育児や介護を成し遂げていくために発揮する力すなわち「家族のケア力」の高まりを明らかにし,家族のケア力を高める看護援助を検討することである。育児に取り組む家族1世帯と介護に取り組む家族2世帯を分析対象とし,筆者自ら保健師の立場で行った援助過程における家族員の言動および筆者の保健師としての言動とそれに伴う認識をデータとした。家族員の言動からケア力に関連する認識と行動を抽出し,その発展的変化から「家族のケア力の高まり」を明らかにした。そしてそれに影響を及ぼした看護援助との関連を調べ,家族のケア力を高める看護援助を検討した。その結果,家族のケア力は「健康生活を維持・獲得していく力」,「児・要介護者の健康問題に対処していく力」,「家族員間の協力関係を築いていく力」,「外部資源を活用していく力」の4項目に整理され,これらは関連性と循環性をもって高められていた。家族のケア力を高める看護援助は,以下が重要と考えられた。1.ケア力を発揮する基となる家族員の認識を深める。つまり,家族員が家族の健康生活や健康問題に対する関心や理解を深めたり,それに対応していこうとする意欲や責任感を高めたりする援助である。2.家族員の対処行動を維持・促進する。つまり,家族員が健康生活の維持・獲得やケアを必要とする家族員の健康問題の改善・解決に向けて好ましい行動や必要に応じた工夫,自信をもって対処できるよう促し,継続を支える援助である。3.家族員のケア力が発揮できる状況を整える。一つは,家族員が協力しあえる家族であることを実感することで,自己の対処意欲や家族内他者を尊重した行動をもたらすよう家族内The purpose of this study was to explore nursing care as a means to expand family self-care. Participants in the study comprised three families, one with a child and two with elderly dependents. Data was collected from the author's own nursing practice as from a public health nurse. Analysis of each family focused on the following: 1) expanding changes in cognitions and behaviors concerning family self-care of family members; 2) nursing actions that might have an effect on expanding changes in cognitions and behaviors concerning family self-care of family members. The following conclusions about nursing care as a means to expand family-self care can be drawn from this study. First, the family member's individual latent ability is enhanced by promoting each member's awareness of his or her health, life-style, and care for children or elderly family members. Second, the care-coping behavior of each member is promoted and maintained. Finally, a situation is created that brings out the family member's individual ability within the family unit by strengthening family interaction and availability of social resources.
