本研究の目的は,在宅要介護高齢者と介護者である配偶者(以下高齢夫婦)は互いにケアしあう存在であるという前提にたって,高齢夫婦のケアしあう関係とケアしあう関係を促進する看護援助を明らかにすることである。作成した[ケアしあう関係を促進する援助の指針]に基づき3人の要介護高齢者とその介護者である配偶者を看護し,その看護経過を詳細に調べ,高齢夫婦のケアしあう関係を促進する看護援助を検討した。結果は以下の通りである。高齢夫婦のケアしあう関係は,〈両者はそれぞれの体力と心にゆとりができ相手をケアする可能性がある〉〈両者は相互理解している〉〈両者は互いに相手の価値を認めている〉〈両者は互いに相手に専心している〉〈両者は互いに相手の成長を助けている〉〈それぞれが相手へのケアを通して自分の生の意味を感じている〉が見いだされた。高齢夫婦のケアしあう関係を促進する援助は,【相手をケアするゆとりを生み出す援助】【ケアする相手への専心を促進する援助】【相互ケアを促進する援助】【相手に対するケアを促進する援助】【相手をケアするゆとりの支援を外部・他の家族に求める援助】に集約された。老化や健康障害により心身のゆとりを失った高齢夫婦の双方に対する【相手をケアするゆとりを生み出す援助】,要介護状態でのつらさや心身のゆとりがない状態での介護などから離れた心を再びつなぐ【ケアする相手への専心を促進する援助】ならびに,老化や要介護状態などで役割を失った夫婦が新たに協働したケアをはじめるよう促す【相互ケアを促進する援助】はすべてのケアしあう関係を促進するのに関連しており,高齢夫婦のケアしあう関係を促進The purpose of this study is to clearly define what is the mutually cared relationship between elderly couples, as well as the type of nursing support to promote such relationship. To this end, I take the standpoint that the elderly people to be cared for at home and their spouses as caregivers (hereinafter called the elderly couple) should cooperate with each other in their nursing care. Based on the "guidelines for the nursing support to promote the mutually cared relationship" that I had prepared, I looked after three elderly individuals requiring nursing care and their spouses (i.e., caregivers) and closely examined their processes of nursing care in order to study and clarify the nursing support to promote the mutually cared relationship between the elderly couple. The results are as follows: The mutually cared relationship already established between the elderly couple has been shown to have the following characteristics: "Both partners have enough allowance or latitude in their minds or physical strength to care for each other"; "both partners understand each other well"; "both parties appreciate each other's values"; "both partners give each other an undivided attention"; "both partners assist each other in their growth"; and "both partners regard caring for the other partner as being something meaningful for their own life." The nursing support to promote the cared relation ship between the elderly couple, specifically represents the "support to help produce enough mental and physical allowance or latitude to care for the other partner"; the "support to promote the undivided attention to the partner to be cared for", the "support to enhance the mutual care between partners"; "support to promote the care for the other partner" and the "support that allows the elderly couple to ask help for out side or other family members in order to care for the partner as free of pressure as possible."
