Many patients with end stage coronary heart disease (CHD) suffer from angina pectoris refractory to standard palliative approaches. Hemorheotherapy remains an alternative, and extracorporeal hemo-rheotherapy deserves consideration. 10 patients (9 male, 1 female, 61 ± 0.9 years old) with refractory CHD were treated over 9 months. For primary separation an ASAHI OP-05 was used. Plasma differential separation was applied at hematocrit levels of 38-42%, using an ASAHI 1760 cascade filter. 2 filtration procedures were performed within 2 days, processing 80-120% of the plasmavolume. Fibrinogen as the main chemical parameter was lowered by 50%. The work capacity increased to about 170%, and anginal attacks as well as nitroglycerin consumption were reduced to less than 50%. The treatments were repeated at intervals of 6 weeks. They were well tolerated. Extracorporeal hemorheotherapy using cascade filtration is effective in CHD patients with otherwise refractory angina. The influence on the prognosis needs to be examined.
