Overlooking a treatment period covering the past 15 years, a total of 37 patients treated at Cologne (10 of them homozygous) and 3 different trials, it is now obvious that visible regression of coronary stenosis due to delipidation can be obtained only in young patients if treatment starts at an early stage of the disease. It appears that a lowering of the serum LDL cholesterol to less than 100 mg/di and improvement of the LDL/HDL ratio are prerequisites. Long-term LDL-apheresis at least maintains the quality of life present at the first apheresis, and delays or even prevents restenosis in patients following PTCA or bypass operation. The strategies for LDL-apheresis are directed towards long-term treatment, agressive delipidation, combinations with drugs and upcoming new therapeutic approaches.
