Dental infection such as pericoronitis of the wisdom tooth is often encountered in dental clinics. However, such infection occasionally spreads, resulting in serious outcomes. We encountered a rare patient who developed an abscess in the temporal fossa after tooth extraction, showed improvement, but subsequently developed a delayed brain abscess extending intracranially and extracranially after a couple of months. She recovered without severe sequelae. Concerning infection in the somatognathic system, anaerobic microorganisms are often causative. Our case suggested that careful observation with consideration for decreased immunocapacity and the appropriate selection of antibiotics after infection are necessary when tooth extraction is performed in elderly and diabetes mellitus patients.
