甘草含有方剤を内服中に高血圧が悪化した71歳女性。血清Kは正常範囲であったが血漿レニン活性(PRA)と血漿アルドステロン濃度(PAC)は低く,偽アルドステロン症が疑われた。甘草含有方剤を中止したところ,悪化した高血圧が改善しPRA,PACの上昇を認めた。PRAの上昇が遅延したためPAC/PRC比は一時的に上昇したが,16週後には低下しベースラインに復帰した。甘草含有処方内服中に血圧上昇を来たした場合,低カリウム血症を認めなくても,偽アルドステロン症を考慮する必要がある。また,経時的なPAC/PRC比の経過観察が偽アルドステロン症の把握に有用であることが示唆された。
