Secondary Acute Leukemia Following Chemotherapy for Ovarian Malignancy

To evaluate the risk of leukemia after chemotherapy for 337 patients with primary ovarian cancer. From July, 1971 to December, 1995, 337 patients with ovarian cancer were treated with various chemotherapy regimens including intravenous chemotherapy without platina-based agents (group 1: 72 cases) and with platina-based agents (group 2 : 226 cases), and intraperitoneal chemotherapy with etoposide and either cisplatin or carboplatin (group 3: 39 cases). The incidence of secondary leukemia experienced after these regimens was evaluated, and the relative risk was calculated. Five patients developed leukemia (1 case in group 1, 2 in group 2 and 2 in group 3). The relative risks of group 2 and 3 for group 1 were calculated as 0.9 and 14.7, respectively. The latent periods were 57 to 63 months in group 1 and 2, and 16 to 25 months in group 3. The type of leukemia was acute nonlymphocytic one except for one case. Remission of leukemia was not experienced. Treatment induced leukemia was experienced not only in the patients treated by alkylating agents (group 1) but also in those by cisplatin, its derivatives or etoposide.
