A 31-year-old male with a case clinical picture of jaundice was also found to have erythropoietic protoporphyria. Clinical diagnosis was expected in view of his history of photosensitivity and was confirmed by high serum levels of erythropoietic protoporphyrin. The patient condition was rapidly deteriorated and died of liver dysfunction, and autopsy revealed micronodular liver cirrhosis was present. Macroscopic and microscopic examination confirmed erythropoietic protoporphyria. The micronodular cirrhotic pattern is specific in figure, and crystals are present in hepatocytes. Involvement of the liver in erythropoietic protoporphyria is discussed.
