Identical twin boys with dysraphic lesions, lumbar meningomyelocele are described. One infant had an associated typical Type II Arnold Chiari malformation. The other had a very rare lesion, almost complete agenesis of the cerebellum. The twins illustrate the variability in expression of a dysraphic defect and the difficulties of a precise definition of the Arnold Chiari malformation. The known etiologies of cerebellar hypoplasia are briefly discussed.
