Electromechanical Interval (EMI) as a New Tool for Monitoring Critical Neonates : 1st report of animal study

PEP is known to be sensitive indicator of cardiac function and hemodynamic status. Electromechanical intervals (EMI) which is the time interval between the beginning QRS complex of EEG and the upstroke of arterial blood pressure, has similar pathophysiological meaning as PEP, though EMI is slightly longer than PEP as much as pulse transmission time at measuring site of blood pressure. Five puppies were used to analyze the change of EMI while they were placed on acidosis, hypoxia, hypovolemia and combined conditions of those. EMI was measured from polygraphic record of ECG and blood prssure. Two puppies were placed on acidosis by administrating NH_4 C1 and their EMI were prolonged 4.77 msec and 5.3 msec per every 0.1 decrease of pH respectively. Two puppies were placed on hypoxia and EMI were prolonged significantly by 8.42 msec and 12.8 msec respcectively. Hypovolemia and acidosis were overlapped on top of hypoxia separately on each cases. EMI was markedly prolonged on the former case and was not significantly change on the latter. One puppy was placed on hypovolemia and his EMI became shorter by 10.8 msec without appereciable change of blood pressure. These studies demonstrated clearly the effect of acidosis, hypoxia and hypovolemia on EMI and proved that EMI is as sensitive indicator of these changes as PEP is. Continuous direct blood pressure monitor and ECG monitor are one of the routine in the management of sick neonate. EMI can measure continuously from these monitors and could be used as a new additive information in the care of sick neonate.
