Marked hypercalcemia and weight loss in a patient with myocardial infarction complicated by psoriasis vulgaris

We recently encountered a patient with myocardial infarction complicated by psoriasis vulgaris who developed marked hypercalcemia and weight loss in response to topical application of calcipotriol (a vitamin D3 derivative known to have little effect on systemic calcium metabolism) during his hospital stay due to myocardial infarction (the patient was admitted one week after the onset of myocardial infarction). Since psoriasis vulgaris is often seen as a complication of diseases managed by internists, the experience with the present case suggests the necessity of paying special attention to serum calcium levels when dealing with patients on vitamin D3 pintment therapy, patients with active disease or patients with compromised renal function.
