A male patient showed a skeletal Class III jaw relationship with anterior open bite and slight skeletal asymmetry at the initial visit. Initially, observation of growth during adlescence was performed due to skeletal Class III family history. Upon reexamination of the patient after 1 year without treatment, the overbite had become more negative presenting a severe anterior open bite and prominent mandibular shift to the left side. After presurgical orthodontic treatment, sagittal split ramus osteotomy was performed when the patient was. Occlusion remained stable with little relapse during 2 years and 6 months follow-up.
