A 47 male patient showed an invaluable clinical course that suggested one of pathological mechanisms in erythromelalgia. This patient was suffered from erythromelalgia of the back first that was improved through 6 sessions of skin temperature biofeedback training, then this symptom shifted to left hand fingers. Although pharmacological treatment, EMG-biofeedback interventions and psychotherapy were not sufficiently effective, balneotherapy worked in this patient. It is suggested that in this patient, erythromelalgia is a kind of abnormal or inappropriate general distribution of the blood pool specially between the skeletal muscle system and the peripheral vascular system that was caused a forced over-distribution in the peripheral capillaries due to excessive muscular tension and squeesing.
