Rheumatic Carditis in Children--A Follow up Study, and An Economical Evaluation : Rheumatic Heart Disease as an Economical Disease

413 cases of rheumatic fever with or without carditis were examined and investigation was made from the prognostic and economical view point. 55 cases with carditis were followed up in the clinic for more than three years after the steroid treatment. Early initiation of adequate amount of steroids seemed to be effective to reduce the incidence of residual cardiac damage. The expenses per day for hospital stay for the treatment of rheumatic fever are shown. Prophylaxis against streptococcus was revealed to be effective to prevent the recurrence of rheumatic fever. The expenses to receive these prophylactic regimen are shown. Some aspects of rheumatic recurrences were investigated, especially its significance on the residual valvular heart disease, and the mime-tic feature of recurrences. The recurrence is most likely to occurr with-in 5 years after the initial attack. Prophylactic method should be carried out on all the rheumatic children, especially with carditis, preferably through their lives, but especially in the first 5 years after the initial attack.
