Surgical Management of Symptomatic Intrasellar Arachnoid Cysts : Two Case Reports

- 論文の詳細を見る
症候性の鞍内くも膜嚢胞2例に対し、中型の症例1にはtranssphenoidal surgeryを、大型の症例2にはcraniotomyを施行し、良好な結果を得た。同部位の手術を行う上で、craniotomyでは、手術侵襲が大きいことが問題となるが、術後合併症の報告はなかった。これに対して、transsphenoidal surgeryは、手術侵襲も少なく非常に有用と考えられるが、術後髄液瘻や視力障害などの合併症が多く報告されており、嚢腫径に応じて手術approachを選択する事と、鞍内のpackingを充分に行うことが、術後合併症の予防に重要と考えられた。
