Cervical Epidural Rhabdomyosarcoma with a Leukemia-like Presentation in an Aged Patient : Case Report

症例は77歳女性。倦怠感、関節痛、左肩から左手にかけての痛みを主訴に某大学病院を受診し、諸検査より急性白血病と診断され、当院血液内科に紹介された。骨髄像では、好塩基性の細胞質と細胞質内空胞をもつ大きな芽球様細胞が認められ、急性白血病やリンパ腫の診断に矛盾しない所見であったが、細胞質にフィラメント様構造をもつ幼弱な細胞が一部観察された。頚椎硬膜外に見つかった腫瘍の摘出術が行われ、免疫染色などからrhabdomyosarcoma(RMS)と診断された。RMSは稀に骨髄転移することがあり、急性白血病のごとく発症することが知られている。鑑別診断をする上で、脳神経外科医も頭の片隅に記憶しておいてよい知識と思われ報告した。
