Gamma Knife Radiosurgery for Acoustic Schwannoma : Early Effects and Preservation of Hearing

28例の聴神経腫瘍患者において、主にMRI上での腫瘍像のsignal、大きさの変化、及ぴ聴力の変化から、聴神経腫瘍に対する比較的低線量でのGamma knifeの効果、副作用について検討した。平均踵鎗周辺線量は12 Gy、最大線量は25 Gyであり、平均観察期間は16ヵ月であった. MRI上腫瘍中心部のsignalの低下、腫瘍の縮小を、それぞれ治療後平均6ヵ月、12ヵ月より認めた. 2例で腫瘍内嚢胞が増大した。聴力保存率は、治療後3, 6, 9, 12, 18, 24ヵ月で、それぞれ85, 80, 72, 75, 60, 50%であった. 一過性の顔面のシビレ、筋力低下が各1例で生じた。聴神経腫瘍の生育を抑え聴力を保存する上で、比較的低線量のGamma knife radiosurgeryが効果的であった。
