Hypoglossal Nerve Paresis Caused by Spontaneous Dissection of Kinked Internal Carotid Artery : Case Report

症例は54歳、男性。突然の頭痛と構音障害をきたし来院。神経学的所見では左舌下神経麻痺を認めた。血管写およびMRI所見より頭蓋外左内頚動脈の解離性病変と起始部の強い屈曲所見を認めた。保存的治療6週後に症状は消失し、動脈解離の画像所見も正常化した。<BR>内頚動脈解離性病変に舌下神経麻痺を合併した報告は数例にみられ、舌下神経が解離性血腫により機械的な牽引と圧迫を受けた結果によるものと考えられている。本症例は解離性内頚動脈に屈曲があり、その病態をさらに強調したものと思われた。
