Acute Traumatic Subdural Hematoma Originating from a Convexity Meningioma : Case Report

既存の円蓋部髄膜腫が出血源となった外傷性急性硬膜下血腫例を報告した。症例は68歳、男性。バイク運転中に転倒し、受傷した。CTで右側頭部に硬膜下、あるいは硬膜外血腫と思われる病巣を認め、緊急手術を行った。手術所見は円蓋部髄膜腫とその直下の硬膜下血腫であり、脳表には出血源は認めなかった。病理診断はangiomatous meningiomaで腫瘍組織内に出血を伴っていた。受傷直前まで意識清明であり、神経脱落症状もなかったことから、頭部外傷により腫瘍組織から出血したものと結論した。明かに外傷に起因する髄膜腫からの出血の報告は本例が最初である。外傷性頭蓋内出血の出血源として、髄膜腫も念頭に置く必要がある。
