3例の三角頭蓋患者の手術例を報告し、本症に対する手術治療の理論的背景と、実際の手術手技について考察した。三角頭蓋児は前額部の突出が特徴的であるが、前頭縫合のみならず前頭蓋底の縫合にも早期融合が存在し、前頭蓋底の形成不全も伴っている。従って、治療に際しては前頭縫合の離断のみでは不十分であり、前頭蓋底の拡大を図る必要がある。諸家によって考案、改良されたlateral canthal advancementとradical forehead remodelingがこの目的に適しており、今回の3例でも満足すべき結果を得た。年長児の方が手術の安全性は高いが、頭蓋形状の正常化、さらに眼窩変形による視機能異常の予防などを勘案し、生後3〜6ケ月の手術が望ましい。
