Immunohistochemical Detection of Tumor Marker in Recurrent Clivus Enterogenous Cyst : Case Report

頭蓋内enterogenous cystが16歳男性に発症し、10年後に再発し興味ある病理所見を呈した症例を報告する。再発時外科的に嚢胞壁を部分切除しOmmaya reservoirを留置し良好な経過を得た。組織学的には、初発時嚢胞壁は一層の扁平〜立方状の細胞で被われ杯細胞の集族も認められた。再発時にはそれに加え、消化管上皮に類似した腺管構造も見られた。免疫組織学的には初回⋅再発時ともにCEA，S-100，EMA，CA19-9が、陽性を呈した。再発時CA19-9の髄液中濃度は高値を示したが血清濃度は正常範囲内であった。enterogenous cystは再発の可能性があることから、可能な限りの外科的切除と、Ommaya reservoirの留置を考慮し、慎重な経過観察が必要と考えられた。
