Removal of Petrous Apex Meningioma and Microvasculor Decompression for Trigeminal Neuralgia through the Anterior Petrosal Approach : Case Report

症例は、典型的右三叉神経痛を患う64才女性。MRI上、長径12mmの右錐体骨尖端部髄膜腫で、三叉神経は腫瘍によって強く内側に圧排されていた。Anterior petrosal approachで手術。三叉神経の圧排変形に加え、上小脳動脈が神経根部内側を圧迫していた。腫瘍を全摘後、神経根部の血管減圧術を施行。三叉神経痛は消失した。三叉神経痛に対する血管減圧術は通常後頭下開頭で行われるが、錐体尖端近傍の腫瘍を伴う場合anterior petrosal approachを用いると、病変部への接近距離が短く他の脳神経に邪魔されず、腫瘍栄養血管を手術初期に処理でき出血が少ないなどの利点がある上に、三叉神経血管減圧術も安全に施行できた。本症例におけるanterior petrosal approachの利点を強調した。
