PROGNOSTIC POTENTIAL OF SERUM CA125 AND PREGNANT MARKERS IN THREATENED ABORTION

切迫流産の臨床管理上, 早期の予後判定は重要である. 本研究では妊娠4週から12週までの切迫流産患者256名(正常児を得た予後良好例166名, 流産に終った90名)を対象とし, retrospectiveに従来より用いられている血中β-hCG, progesterone, cortisol, HPL, E_1, E_2, E_3, 尿中hCG, Eの測定および, 超音波診断法による胎児心拍の確認に加えて血中CA125を測定し, 血中CA125測定が切迫流産の予後判定に利用できる可能性を検討した. その結果, 血中CA125値は妊娠6週に最高値を示し, 妊娠7週より漸減することが予後良好例で認められた. 従来用いられてきたホルモン値は妊娠6週以後で予後良好例が有意に高値を示すことが認められ, 超音波検査は妊娠8週以後で有効であったが, 血中CA125値は妊娠4週という非常に早期から妊娠8週までは予後良好例が予後不良例より有意に高値を示した. 以上により血中CA125値の測定は妊娠4週から切迫流産の予後判定に有効であることが示唆された.
