STUDIES ON SERUM CA125 LEVELS IN PREGNANT WOMEN

CA125は胎生期体腔上皮に存在する糖蛋自と関連性を有していると報告されている.今回,妊娠4週より妊娠42週の各時期の健常妊婦234例,産褥5日目の健常褥婦40例,異常妊娠14例(子宮内胎児死亡12例,胞状奇胎2例)の妊娠性変化に伴う母体血清中のCA125値について検討し,以下の結果を得た. 1)CA125値は妊娠初期で最も高く65.0±77.3U/ml,陽性率(>39U/ml)59.8%であり,妊娠中期で平均22.3±10.6U/ml,陽性率11.1%と最も低値を示し,産褥期で33.2±24.7U/ml,陽性率25.0%と少し上昇傾向を示した. 2)子宮内胎児死亡(IUFD)および胞状奇胎症例の血清CA125値は全例陽性値を示し,IUFDは平均392.5±275.8U/ml,胞状奇胎は65.0±15.0U/mlであつた.この異常妊娠症例のCA125値はD&Cあるいは分娩後はすみやかに減少し,2週〜4週後に非妊時のレベルに戻つた. 以上より,本邦婦人の正常妊娠経過に伴う母体血清CA125値の変化が確認出来た.さらに,CA125を卵巣腫瘍マーカーとして利用する際に,妊娠の影響を考慮する必要があるとともに,妊娠異常,特に胎児死亡の血清レベルでの補助診断に利用出来る可能性も示唆された.
