BIOPHYSICAL PROFILE AND ITS RELATION TO FETAL BLOOD GAS LEVEL OBTAINED BY CORDOCENTESIS

• Biophysical Profile Score (BPPS) が胎児の血液ガス値をどの程度反映するか調べた。胎児採血が必要であった妊娠30週未満55例, 妊娠30週以降95例の150例の胎児に対して胎児採血直前にBPPSを評価した。妊娠30週以降の胎児のBPPSを3群に分けて検討したところ, その分布は満点の12点が39, 8〜11点が35, 7点以下が21例であり, 妊娠30週未満では各々8, 27, 20例であった。これらにつき〓帯静脈穿刺による胎児血pH, pO_2, pCO_2とBPPSとの関係を検討した。BPPSの3群間で胎児血ガス値のいずれも有意の差は認めなかった。すなわち, BPPS 8点以上の胎児状態良好と考えられるなかにも胎児血液ガス値が不良のものも存在し, 7点以下の胎児でも血液ガス値が良好であるものも存在した。特にIUGR胎児で, BPPSが8点以上であるにもかかわらずsevere hypoxemiaと考えられるものが存在していた。したがって, 胎児がIUGRなどの異常がある場合はBPPS以外に胎児採血により胎児の状態を把握する必要がある。
