A CASE-CONTROL STUDY OF UTERINE ENDOMETRIAL CANCER IN JAPANESE AND FINN

最近日本婦人における子宮内膜癌の増加傾向が指摘されているが北欧の婦人の約1/4の発生率である. このように明らかに異なる発生率の原因として社会的環境因子, 遺伝的因子の相違などが考えられる. そこで内膜癌の発生頻度が高い国に属するフィンランド婦人と日本婦人とを治療期間を同一にして, 内膜癌の症例を選びそれぞれの症例に年齢と治療期間をマッチさせた子宮頚癌ならびに良性婦人科疾患の症例を抽出した. これらの症例の種々の個人的又は臨床的な特徴の16項目について比較した. 検討した症例は内膜癌がそれぞれ50例で子宮頚癌ならびに良性疾患群49例, 合計296例である. 1.内膜癌の症例群ではフィンランド婦人の方が初回分娩と最終分娩年齢が高いことと肥満度がより強いことが日本婦人と異なるほかは類似し, とくに少妊, 少産, 未婚婦人が多く認められた. 2.子宮頚癌の症例では両国婦人の間で日本婦人の方が妊娠回数が多いほかはすべての項目に類似性が認められた. 3.良性疾患の症例群ではフィンランド婦人に内膜癌の患者と個人的, 臨床的事項の類似性が認められた. このように良性疾患群と内膜癌症例群の検討事項の類似性からフィンランド婦人では内膜癌のリスクファクターを有する婦人が日本婦人より多いものと考えられる. とくに閉経後の肥満婦人が多く, このような因子が両国の内膜癌発生率の差の一つと考えられた.
