IMMUNOHISTLOGICAL IDENTIFICATION OF T AND B CELLS IN NORMAL AND MALIGNANT TISSUES OF THE UTERINE ENDOMETRIUM

子宮体癌61症例の癌増殖局所におけるTおよびB細胞の癌細胞への関与について, およびこれらの細胞の浸潤の程度と種々の因子との関係について酵素抗体法を用いて検討した。年齢別に検討すると40歳台の体癌患者11例中9例はT, B細胞の浸潤が少なく, 若年者の癌患者の発生には免疫監視機構の障害が関与している可能性が示唆された。正常内膜で多数のT細胞の浸潤をみる症例は7.4%だが, 癌組織では45.9%を占め, 癌組織に多くのT細胞の浸潤がみられた。また, 同一子宮から正常内膜と癌組織の両部分を観察しえた症例では, 前者では少数のT細胞浸潤をみるのみであったが後者には多数のT細胞浸潤が観察され, これらのことはT細胞の癌細胞認識を示唆するものである。進行期別に検討するとI期, II期に比較しIII期, IV期ではT細胞浸潤は少ない傾向がみられ, 進行癌ではT細胞反応が弱く免疫応答が低いことが推察された。組織分化度とT細胞浸潤の程度の間には相関は認められなかったが, 癌の深層浸潤症例や癌病巣が大きい症例ではT細胞浸潤は少ない傾向がみられ, 進展例では免疫応答が低いことが示唆された。以上のごとく癌増殖局所におけるT細胞浸潤は生体防御機構の免疫応答の一つと考えられ癌の進行への関与が示唆された。
