SAFETY OF CORDOCENTESIS UNDER ULTRASOUND GUIDANCE FOR FETAL BLOOD SAMPLING

超音波断層法直視下での臍帯穿刺による胎児採血を試み, その安全性とリスクについて検討を加えた. 43例を対象に計45回の臍帯穿刺を行った. 43例の内訳は非免疫性胎児水腫23例, 胎児奇形12例, 染色体異常3例, Rh式血液型不適合妊娠3例, 特発性血小板減少性紫斑病2例であった. 胎児採血施行時の妊娠週数は妊娠17週から39週であった. 45回中42回(93.3%)で胎児血採取に成功した. 合併症については, 穿刺後の羊水腔への出血が17回(37.8%)で認められた. 出血を認めた17回中12回(70.6%)では, 出血持続時間は2分以内であった. 5分以上出血が持続した1例は胎児水腫の1例で, 11分間の持続出血があり, 胎児徐脈のため緊急帝王切開術で児を娩出した. 他の44回では胎児心拍数モニターの異常は認めなかった. 破水, 早産, 胎内感染, 胎児死亡はなく, 分娩後の胎盤および臍帯の損傷は認められなかった.
