EVALLUATION OF AMNIOCENTESIS GUIDED BY REAL-TIME ULTRASONIC TOMOGRAPHY

1978年10月から1981年7月まで,九大産婦人科でreal-tine超音波断層法を用い直視下に行った羊水穿刺250例を対象とし,その適応,成功率,安全性について検討し以下の結果を得た.1.羊水穿刺の適応は,染色体検査160例(64%),胎児奇形診断37例(14.8%),胎児成熟度診断37例(14.8%),Rh不適合妊娠16例(6.4%)であった.施行時期は,妊娠前・期19例(7.6%),中期151例(60.4%),後期80例(32%)であった.2.成功率は250例中246例,98.4%であり,失敗例4例は全て羊水過少症合併例であった.第1回穿刺での成功率は86.6%で,妊娠前期では89.5%,中期で86.7%,後期で83.4%と妊娠時期による有意な差はみられたかった.3.bloody tapは,13例(3.6%)にみられた.5例は経胎盤穿刺例,8例は非経胎盤穿刺例であった.経胎盤穿刺28例中のbloody tap率は17.9%で,非胎盤穿刺例での3.6%に比べ有意に高値であった.4.羊水穿刺抜針後,羊膜腔内への出血が16例で認められ,その7例は経胎盤穿刺で9例は非胎盤穿刺例であった.最も長い出血時間は経胎盤穿刺例での6分であった.出血時間は経胎盤穿刺例で長く,120秒以上続く出血は非経胎盤穿刺例では認められたかった.胎盤穿刺例では60秒以内に止血する例はなかった.
