PVB (Cisplatin, Vinblastine, Bleomycin) 療法が奏効した卵巣原発卵黄嚢癌併合奇形腫の1例

We reported a case of combined teratoma with yolk sac tumor which was successfully treated with cisplatin, vinblastine, and bleomycin (PVB) after surgey. A seven-year-old girl was referred to our hospital for evaluation of abdominal mass. On physical examination a huge mass occupied the pelvis was palpable. A CT scan revealed a large mass with internal calcification at the lower site of abdomen. A serum α-fetoprotein (AFP) level was elevated at 4,508 ng/ml. On laparotomy, a right ovarian sdlid tumor with 800ml of bloody ascites and disseminated milliary tumors in the retroperitonium were found. Right salpingoophrectomy and hysterectomy were performed. The tumor weigt was 600grams and the size was 12 X 17 X 18cm. The pathological findings was compatible with combined teratoma with yolk sac tumor and the stage of III a. Following surgery, she was treared with four courses of PVB therapy. The serum AFP level had returned to normal after three courses of PVB therapy. She had maintained by monthly injection of vinblastine for two years and has been doing well with no evidence of recurrence and no rise in the serum AFP at 25 months postoperatively. PVB therapy following surgery should be required for successful treatment in patiens with yolk sac tumor of ovary.
