A POS-Problem oriented system-based nutrition guidance evaluation form, which allows forintervention of education focusing on awareness and behavioral changes of patients following nutrition guidance, and a self-evaluation form, which is completed by patients, were introduced.1. The introduction of the self-evaluation form resulted in the following: (1) regarding eating habits related to overeating, it was revealed that there were a significantly large number of false reports, and based on these falsifications, the recognition and modification of nutrition problems were able to be effected more precisely; (2) it was found that the gap between those in charge of nutrition education and patients was large in terms of goal setting, and thus the accuracy of nutrition evaluation conducted only by personnel in charge of nutrition education was subsequently improved; and (3) patients were able to be evaluated more precisely based on evaluations of nutrition education.2. By setting reference and problem values in nutrition evaluation, each item of the nutrition education plan could be standardized for individual patients.3. The POS-based nutrition guidance evaluation form allowed doctors and other medical staff to easily obtain patient information, including nutrition condition, effects of nutrition education, and nutrition problems, through patient case records.
