Problems in deriving soft-tissue thermal index (TIS) using spatial-peak intensity are investigated by simulations of ultrasonic fields radiated from circular transducers. TIS gives information about the temperature rise in soft tissues for the safety evaluation of diagnostic ultrasonic equipment. To estimate the depth of highest temperature rise properly in the TIS derivation process, it is necessary to reduce the break-point depth of spatial-peak intensity measurement to the point close to the transducer. However, if TIS remains to be derived from spatial-peak intensity, the depth cannot be estimated and the overestimation of TIS is revealed by reducing the break-point depth. It is shown that TIS should be derived using spatial-average intensity in the near field to overcome these problems.
