ANTIMICROBIAL ACTIVITY OF MEROPENEM AND ITS CLINICAL EFFICACY IN URINARY TRACT INFECTION

We studied the antimicrobial activity of a new carbapenem antibiotic, meropenem (MEPM), and its clinical efficacy in urinary tract infection (UTI).<BR>1. MEPM had excellent antimicrobial activity against both of gram-positive cocci and gram-negative rods in 20 reference strains tested in this study. MICs of MEPM were 1.56μg/ml or lower. Against the strains of gram-positive cocci isolated from UTI, the antimicrobial activity of MEPM was equal or inferior to that of imipenem but superior to those of ceftazidime, cefuzonam and ceftizoxime. Although the antimicrobial activity of MEPM was no higher than the other antibiotics against the clinical strain of Serratia marcescens, it was the highest against the other clinical isolates of gram-negative rods (<I>Escherichia coli, Citrobacter freundii, Klebsiella pneumoniae, Enterobacter cloacae and Pseudomonas aeruginosa</I>).<BR>2. In 10 patients with complicated UTI treated with 0.25 g of MEPM twice a day fOr 5 days, the therapeutic efficacy was evaluated according to the criteria proposed by the Japanese UTI Committee, and the overall efficacy rate was 50%. In 13 patients treated with 0.5 g of MEPM twice a day for 5 days, the therapeutic efficacy in 11 patients was evaluated according to the same criteria, and the efficacy rate was 91%. Of the 21 patients treated with MEPM in this study, the clinical efficacy was excellent in 9 patients, moderate in 6 patients and poor in 6 patients, an overall efficacy rate of 71%.<BR>3. Of 23 patients treated with MEPM, 4 patients showed transient elevations of serum enzymes (ALP, LDH, GOT, GPT and γ-GTP).<BR>In the remaining 19 patients, there were no significant clinical or laboratory adverse reactions.<BR>Therefore, MEPM has been demonstrated to be an effective and safe drug in the treatment of complicated UTI.
