BASIC AND CLINICAL EVALUATION OF A CARBAPENEM, MEROPENEM

We conducted basic and clinical evaluation of the carbapenem meropenem (MEPM). The <I>in vitro</I> postantibiotic effect (PAE) of MEPM against <I>Klebsiella pneumoniae</I> BK was 1.0 h and 1.2 h at 2 MIC and 4 MIC, respectively. That of imipenem (IPM) was 0.7 h and 1.2 h at 2 MIC and 4 MIC, respectively. The in vivo effective regrowth times (ERT) of MEPM, MEPM/cilastatin (CS), 1PM and IPM/CS against <I>K. pneumoniae</I> BK were 5.1, 9.5, 2.7 and 5.3 h, respectively. The ERT of MEPM and IPM against <I>Pseudomonas aeruginosa</I> ATCC 94966 were 11.7 and 10.7 h, respectively.<BR>MEPM was clinically evaluated in 6 patients: 4 with pneumonia, 1 with acute bronchitis and 1 with chronic respiratory tract infection. The isolated organisms were <I>Streptococcus pneumoniae</I> in one case, Streptococcus sanguis in one case and mixed infection of <I>P. aeruginosa, Klebsiella oxytoca, Alcaligenesxylosoxydans</I> and <I>Xanthomonas maltophilia</I> in one case.<BR>The clinical efficacy was excellent in 1 case and good in 5 cases. No side effects or abnormal laboratory findings were observed.
