Clinico-pathological assessment of cefazonam (CZOM) concentrations in bile and gallbladder tissue.

Penetration of cefuzonam (CZON) into the gallbladder tissue and bile was examined after intravenous drip administration at a dose of 1 g. The results were classified by the presence or absence of obstruction of the cystic duct and the degree of inflammation.<BR>In the group without cystic duct obstruction, the mean bile level of CZON was 3, 008. 8μg/ml in a patient with slight, 1, 531.8μg/ml in one with moderate, and 2, 258.5μg/ml in one with severe inflammation. Irrespective of the degree of inflammation, a high concentration of CZON penetrated. The mean concentration of CZON in the gallbladder tissue was 226.2μg/g in the patient with slight, 139.2μg in the patient with moderate, and 26.5μg/g in the patient with severe inflammation. The penetration was excellent in the patients with slight or moderate and good in the patient with severe inflammation.<BR>In the group with obstruction of the cystic duct, all patients had severe inflammation. Although the mean bile level of CZON was low (2.53μg/ml), the mean concentration in gallbladder tissue was good (22.1μg/g), being similar to that of the patient without obstruction of the cystic duct with severe inflammation (26.5μg/g).
