We investigated the efficacy and safety of cefdinir (CFDN), a new oral cephem, by administering it to 18 patients with acute respiratory tract infection.
The subjects were 11 males and 7 females ranging in age from 31 to 80 years. The drug (100 mg) was given orally in one capsule three times a day after each meal for 4 to 14 days consecutively.
The overall efficacy of CFDN was good in 10, fair in 2 and poor in 1 of the 13 patients with bronchitis, good in all the 3 patients with bronchopneumonia, and excellent in I and fair in 1 of the 2 patients with pneumonia. The efficacy rate was 77.8%.
Bacteriologically, causative organisms from the 12 patients were identified before the beginning of the study. Haemophilus parainfluenzae was eradicated in 4 and persisted in 1 of 5 strains.
Haemophilus parahaemolyticus was eradicated in 3 and replaced in 1 of 4 strains. Klebsiella pneumoniaepersisted in 1 and was replaced in 1 of 2 strains. Two strains of Streptococcus pneumoniae and one each of Klebsiella oxytoca, Enterobacter cloacae and Enterobacter agglomerans were eradicated. The eradication rate for all the organisms was 89.5%.
No subjective or objective side effects were noted in any of the patients. A slight increase in GOT and GPT was observed in only one patient, but the values returned to normal after the end of the study.
We consider CFDN to be useful in the treatment of acute respiratory tract infection.
