EFFECT OF CEFDINIR ON INTESTINAL MICROFLORA IN HEALTHY VOLUNTEERS

We investigated the effect of cefdinir (CFDN) on the intestinal microflora of 6 healthy male volunteers before, during and after oral dosing with 200 mg 3 times a day for 15 consecutive days, and the relationship between the drug concentrations in feces and its β-lactamase activity. Another three volunteers were used as controls. The results from the 6 volunteers were divided into the following 3 groups:(1) Anaerobic bacteria decreased markedly because of high concentrations of the drug in the feces.(2) Anaerobic bacteria decreased transiently because the drug was present in the feces early in the dosing period, and thereafter the drug was not detected in the feces with the increase of, β-lactamase activity later in the dosing period.(3) Anaerobic bacterial counts scarcely changed because β-lactamase activity was always present. Therefore, bacterial counts in the feces were affected by the relationship between the residual activity of the drug and β-lactamase activity. Total aerobic bacterial counts scarcely changed in all cases. <I>Bacteroides, Bifidobacterium, Clostridium, Eubacterium</I> and <I>Escherichia coli</I> were markedly reduced by CFDN. The duration required to return to the original microbial flora was considered to be 7 days or longer after the last dosing. The CFDN-reSlstant strains tended to increase during dosing and then decrease with a gradual return to normal. Feces, in which <I>Clostridium difficile</I> was detected in 3 of the CFDN group and in one of the control group, were normal.
