BAY o 9867, which was synthesized by Bayer A. G., is a quinoline-carboxylic acid derivative antimicrobial for oral administration. Antibacterial activities and clinical effects of BAY o 9867 were examined in this study.<BR>The results are summarized as follows:<BR>1. The antibacterial activity of BAY o 9867 was compared with those of ENX and OFLX. Each 25 strains of clinically isolated <I>S. aureus, S. epidermidis, E. faecalis, P. aeruginosa, P. mirabilis, E. coli, K. pneumoniae, S. marcescens</I> and <I>H. influenzae</I> were inoculated in the inoculum size of 10<SUP>6</SUP>cfu/ml.<BR>It was showed that BAY o 9867 was superior to ENX against all the strains.In comparison with OFLX, BAY o 9867 showed higher antibacterial activities than OFLX except for <I>S. aureus</I>.<BR>2. BAY o 9867 was used in the treatment of 26 cases of respiratory infectious disease: 12 cases of acute bronchitis, 5 cases of acute tonsillitis, 3 cases of pneumonia, each 2 cases of chronic bronchitis and diffuse panbronchiolitis and each one case of acute pharyngitis and secondary infection of bronchiectasis. BAY o 9867 was administered orally at the doses of 400-600mg divided into two or three times for 3-15 days.<BR>The clinical effects were excellent in 4 cases, good in 15 cases, fair in 3 cases and poor in 3 cases and one case with acute bronchitis was not evaluated from discontinuation of treatment caused for adverse reaction.Causative organisms were eradicated in 7 cases out of 12 in which they had been isolated before administration.<BR>As for adverse reaction, nausea and anorexia were observed in one case.No abnormality was recognized in the laboratory findings.<BR>From the above results, BAY o 9867 is considered to be a useful drug for respiratory infectious disease.
