FUNDAMENTAL AND CLINICAL STUDIES ON MT-141 IN UROLOGICAL FIELD

A new semisynthetic cephamycin, MT-141, was studied both fundamentally and clinically.<BR>MICs of MT-141 as well as four other antibiotics (CMZ, LMOX, CBPZ and CEZ) were determined for 399 clinical isolates stocked at our department. Then, 6 healthy adults were given a single intravenous dose of 1 g MT-141, CMZ or CTX in a crossover manner and changes in serum levels and urinary recovery rate were investigated. In addition, MT-141 was used in the treatment of 9 cases of chronic complicated cystitis in order to determine its therapeutic efficacy.<BR>1) Similar to LMOX and CBPZ, MT-141 showed high activity against <I>E. coli</I>, <I>K. pneumoniae</I> and <I>Indole</I> (+) <I>Proteus</I> sp. 2) Against <I>S. marcescens</I>, <I>E. cloacae</I> and <I>C. freundii</I>, LMOX and CBPZ showed higher activity. 3) From results of absorption and excretion, <I>T</I> <SUB>1/2</SUB>(β)s of MT-141, CMZ and CTX were 1. 8, 1.0 and 0. 9 hr, respectively. The urinary recovery rates within the first 8 hr were 78.5, 86. 9 and 58.6%. Antibiotic concentrations were determined by bioassay method and HPLC, with relatively corresponding results. 4) Out of 9 clinical cases, 7 cases were assessed of clinical results by UTI drug efficacy evaluation criteria. The results were rated as excellent in 2 cases, good in 3 cases and poor in 2 cases, with an efficacy rate of 71%. Bacteriologically, 10 out of 13 (77%) strains were eradicated. 5) Throughout our study, no side effects developed, although as abnormal laboratory test results related to MT-141 an elevation in Al-P and an increase in platelet count were observed in 2 cases. 6) From these results MT-141 is considered to be a drug useful enough in the treatment of urinary tract infections.
