This paper is to report some evaluations made on a newly developed cephalosporin injection, cefmenoxime (CMX, SCE-1365).<BR>1. When the <I>in vitro</I> antibacterial activity of cefmenoxime against clinical isolates of the surgery ward was evaluated, <I>Escherichia. coli</I> and <I>Klebsiella pneumoniae</I> were found to be very sensitive to cefmenoxime, and the sensitivity distribution of each had a peak at 0.20μg/ml and under. In sensitivity distribution of <I>Pseudomonas aeruginosa</I>, peaks were at 50 and 100μg/ml.<BR>2. Blood concentration levels in cases treated with 0.5 g by intravenous injection were 48μg/ml after 15minutes, 9.5μg/ml after 1 hour, and 1.0μg/nil after 4 hours. In cases treated with 1.0g by intravenous injection, the levels were 81μg/ml after 15 minutes, 23μg/mt after 1 hour, and 2.6μg/ml after 4hours; thus in each case, relatively high blood concentration levels were observed.<BR>3. Cefmenoxime of 0.5 g was intravenously injected to 4 patients with external biliary fistulas, and the ceftnenoxime's concentrations in bile were determined. Very high levels were obtained in 3 cases. In a case of bile stagnation with strong obstruction mechanism, cefmenoxime's transfer to bile was not satisfactory.<BR>4. Cefmenoxime was administered to 8 cases of surgical infections, and its clinical effect was excellent in 3 cases, good in 3 cases, fair in 1 case, and poor in 1 case. Any noteworthy side effects were not observed.
