Laboratory and clinical investigations were carried out with Sisomicin and the following conclusions reached:<BR>1. Antibacterial spectrum: Sisomicin has an excellent antibacterial activity against gram-positive bacteria, except <I>Streptococcus</I> and gram-negative bacteria.<BR>2. Antibacterial activity against bacteria isolated from surgical specimen: Excellent antibacterial activity against <I>Staphylococcus aureus, E. coli, Klebsiella pneumoniae, Pseudomonas aeruginosa</I> and others. Against <I>Pseudomonas aeruginosa</I>, Sisomicin was found to be superior to Gentamicin.<BR>3. Serum leveles and urinary excretion: In healthy adults, mean peak serum level of 4.5 μg/ml was reached within 30 minutes after intramuscular injection of 50 mg.(cup-plate method using Moni-trol serum as standard solution).<BR>Urinary concentrations were highest within 1 hour after administration, averaging 167.0 μg/ml. Meanurinary recovery was 58.7% within 6 hours.<BR>4. Tissue distribution: Twenty mg/kg applied intramuscularly to SD rats (n=3), gave highest drug concentrations in the kidney. Serum, lung, heart and spleen contained the drug in decreasing amounts.<BR>In the liver and brain only traces were measured.<BR>5. Results of clinical trial: Sisomicin was applied to 8 patients with surgical infections. Results were "good" in 2, "fair" in 1 and "poor" in 5 cases.<BR>Adverse drug reactions were not observed or reported.
