CLINICALLY EXPERIMENTAL STUDIES ON FOSFOMYCIN

Fosfomycin is a new antibiotic which has very different structure and antibacterial actions from other antibiotics. We examined clinical appliance of fosfomycin to the treatment of chronic respiratory infections caused by Pseudomonas aeruginosa. MIC values of 21 strains of respiratory pathogenic Pseudomonas aeruginosa against fosfomycin ranged from 1.56 to 12.5 μg/ml, and have only one peak. There were no resistant strains before use of fosfomycin. After oral administration of fosfomycin calcium 40 mg/kg in rats, tissue concentrations reached peak values in 30 minutes to one hour. Tissue levels in order of concentration are kidney, lung, serum and liver. 2 cases of chronic bronchiolitis caused by Pseudomonas aeruginosa administered fosfomycin sodium 1.0 g intravenously in 5 minutes. The half life of fosfomycin in serum is one hour to one and a half hours. Sputum levels reached peak values in about 3 hours, and peak values were 3.5-5.0μg/ml, which were about 5% of peak values in serum. In both cases, sputum levels did not reach the MIC values of their pathogenic organisms. Soon after the administration of fosfomycin, those pathogenic Pseudomonas aeruginosa changed into resistant strains. Ordinary dose of fosfomycin is not sufficient to treat chronic respiratory infections caused by Pseudomonas aeruginosa by itself.
