Fosfomycin is a new antibiotic which has very different structure and antibacterial actions from other antibiotics.<BR>We examined clinical appliance of fosfomycin to the treatment of chronic respiratory infections caused by <I>Pseudomonas aeruginosa</I>.<BR>MIC values of 21 strains of respiratory pathogenic <I>Pseudomonas aeruginosa</I> against fosfomycin ranged from 1.56 to 12.5 μg/ml, and have only one peak. There were no resistant strains before use of fosfomycin.<BR>After oral administration of fosfomycin calcium 40 mg/kg in rats, tissue concentrations reached peak values in 30 minutes to one hour. Tissue levels in order of concentration are kidney, lung, serum and liver.<BR>2 cases of chronic bronchiolitis caused by <I>Pseudomonas aeruginosa</I> administered fosfomycin sodium 1.0 g intravenously in 5 minutes. The half life of fosfomycin in serum is one hour to one and a half hours. Sputum levels reached peak values in about 3 hours, and peak values were 3.5-5.0μg/ml, which were about 5% of peak values in serum. In both cases, sputum levels did not reach the MIC values of their pathogenic organisms. Soon after the administration of fosfomycin, those pathogenic <I>Pseudomonas aeruginosa</I> changed into resistant strains.<BR>Ordinary dose of fosfomycin is not sufficient to treat chronic respiratory infections caused by <I>Pseudomonas aeruginosa</I> by itself.
